血糖控制不佳，医生的治疗提议，为何让他望而却步？

新型系统化本品 GLP-1RA 药剂液，双蛋白、多靶点，有望破局

2021 年 10 月底 26 日，国家药品监督管理局（NMPA）批文了德谷本品利拉鲁胺药剂液（IDegLira）在之前国的母公司注册。作为全球首个系统化本品 GLP-1RA 药剂液，IDegLira 是一种全新程序的降糖药器皿，现今已被写入《之前国2型白血病防治指南（2020 年版）》 [6] 。

在抗生素之前，德谷本品和利拉鲁胺可维持各自可溶、稳定的多聚体结构上。 皮下药剂后，两种主要组份体现各自的延长功用程序：德谷本品自我聚合为多六聚可达链并与钙种系统融合，利拉鲁胺可维持七聚体结构上并与钙种系统融合。 经血液循环重回各靶器官后，两种组分与各自蛋白融合体现各自的调节代谢功用（左图 1）。

IDegLira 的诞生以及它的 DUAL 系列 III 期临床深入研究，让我们在茫茫迷雾之前想到了白血病很高血压的希望（左图 2）。

1）胰岛素支配可算率很高[7-15]

不论既往抗生素降糖药器皿疗程还是共同系统化本品或 GLP-1 蛋白激动剂疗程，在调整为 IDegLira 后，与对照组相对来说于，IDegLira 疗程组 HbA 1c 下降可将近 1.9%，HbA 1c

2) 诱发死亡率高于 [8,11-13]

IDegLira 避免诱发风险较高于（0.16～3.5 事件/PYE），相对来说于于甘精本品诱发风险下降57%（DUAL V 深入研究）；与系统化本品或系统化-餐时本品组相对来说于，诱发死亡率减小可将近 50% 以上。

3）体型得益明确[7-11]

既往接受抗生素降糖药器皿疗程的 T2DM 很高血压，运用于 IDegLira 疗程后，体型减高于可减缓或出新现体型减小；其之前，DUAL V 深入研究发掘出新，既往系统化本品疗程的 T2DM 很高血压转换为 IDegLira 疗程后，最少体型变化相差将近 3.2 kg。

4）设计方案简便，依从性很高[14,16]

DUAL VII 深入研究之前，对 T2DM 很高血压完成自身健康量表评估、疗程相关负面影响评分等。 %-，IDegLira 一天一次，可在一天之前的任何时间完成给药，药剂时间有效率，且不受进餐负面影响。相对来说于较于传统意义系统化-餐时本品多次药剂，IDegLira 减少药剂次数，同时本品剂量减半，仍能有效支配6台胰岛素，很高血压疗程关注度来得很高。
